The Kesei line from Nippori to Narita town (or vice versa) only costs 750 yen and takes 1hour 15 mins. Nippori is only 1 stop away from Ueno. It's 1000¥ to the airport itself.
The Keisei Skyliner takes about 50 minutes from Narita (airport) to Ueno (some also head into Shinjuku) and costs around 2500¥ each way. To Tokyo station there is the Narita Express, run by JR. It costs about 5000¥ and takes less time.

The train from the main Nagoya train station to the airport is on the Meitetsu line. It operates an express service for ¥350 more than the regular ¥850. It's easy to find as there are signs in the station with an airplane drawn on it to give you info as to which track the train is on. The train lands you in the terminal itself and all you do is come out and go through the turnstiles and you are ready to check in.
